Medical Information: Personal information of a medical nature, such as medical diagnoses, existing diseases and/or medical issues or emergencies may be shared with TBC Staff which need to be aware of the information or situation. This may include (but not limited to) the TBC President, Resident Supervisors and Dean of Students.
Medical Emergencies: In case of medical emergencies, I authorize TBC and its Faculty & Staff to take necessary measures, such as calling a Doctor, 911 or other medical professional for assistance. I additionally authorize TBC and its Faculty & Staff to provide transportation to a medical facility if the need arises. I authorize professional emergency medical treatment for myself should the need arise while I am under the supervision of Texas Bible College. I understand and agree that TBC does not provide health insurance coverage for me and that if I desire health insurance coverage, I am responsible for obtaining and paying for such insurance.

Food and Medication Allergies: Personal information of an allergy nature, such as food allergies, medication allergies or other major allergies may be shared with TBC Staff which need to be aware of the information. This may include (but not limited to) the TBC President, Resident Supervisors, Cafeteria Director, Cafeteria Workers and Fellow Students which live close to the student.
Learning Disabilities: Personal information of a learning nature, such as diagnoses of ADD, ADHD, Dyslexia or other learning disabilities may be shared with TBC Staff which need to be aware of the information or situation. This may include (but not limited to) the TBC President, Educational Chairs, Assistant Provost, Instructors and Adjunct Faculty.
Educational Information: Personal information of an educational nature, such as grades, add/drop of classes, absences, or other information related to the student’s educational experience at TBC may be shared with TBC Staff which need to be aware of the information. This may include (but not limited to) the TBC President, Assistant Provost, Educational Chairs, Instructors and Adjunct Faculty.

I understand the risk(s) involved in participating in school sponsored events while attending Texas Bible College. I understand that serious injury, and even death, is possible in such participation and choose to accept all responsibility for my safety and welfare while participating in, and/or attending Texas Bible College events. With full understanding of the risk(s) involved, I release and hold harmless Texas Bible College as well as individuals against whom I compete of all responsibility and liability for any injury or claim resulting from such participation and agree to take no legal action or mishap involving participation. I understand that the authorization and rights granted herein are voluntary and that I may revoke any or all of them at any time by submitting said revocation in writing to Texas Bible College. However, by doing so, I understand that I will no longer be eligible for participating in any events sponsored by Texas Bible College.
